Here at Hope Home Care, we care for more than just our patients, we also care for the family members who support them. Our Family Caregiver Support Program was created to aid those family caregivers in managing the emotional, physical, and practical challenges of caring for a loved one at home.
This program provides coordinated support through trusted community-based organizations (CBOs), non-profit partners, government-funded support systems, and internal Hope Home Care services.
Behind every caregiver service is a trained, compassionate care team. Our staff has received specialized training to recognize caregiver stress, assess family needs, and connect caregivers to the right supports at the right time. Our team follows a coordinated referral process to ensure caregivers receive timely follow-up, personalized guidance, and ongoing support throughout their care journey.
Families enrolled in the Caregiver Support Program will receive a Family Caregiver Welcome Packet designed to make support easy and accessible from the start. This packet provides clear, helpful information that caregivers can reference at any time, and helps connect them to training, resources, and support services.
Taking care of someone you love can be rewarding, but it can also wear you out. Our program aims to ensure that caregivers like you feel seen, supported and less alone. We partner with trusted local groups to find you support circles where people speak your language and understand your situation. We offer referrals for behavioral and emotional support services to learn how to lower stress and set healthy boundaries, access grief support, and find local group support services.

Everyone needs a break to recharge, especially when caring for a loved one. Our program connects you with respite care services, which provide temporary relief so you can handle your own appointments, run errands, or simply rest. We’ll use a quick questionnaire to check your stress levels, which helps us better understand and match you with the support that best fits your needs.
A Community Health Worker / MSW can provide screenings to best help you find community resources. These resources include help navigating benefits, assistance with Medicaid applications, information about local Food Pantries, and connections to outside local organizations that provide assistance with mental health, transportation, and housing issues or concerns. These screenings use questions developed by the Center For Hope Services (CFHSI) program.
Taking care of a family member is a big job, but feeling confident in your skills makes a huge difference for both you and your loved one. Our goal is to give you the tools and support you need to feel ready for anything. Through our training program, you can earn a certificate while learning important tasks like how to move someone safely, how to stop the spread of germs, and how to prevent falls. You’ll also learn how to handle emergencies and how to communicate better with doctors and nurses. We also offer “Caregiver 101” workshops that focus on daily routines and how to spot new health problems. These classes are flexible, so we can focus on the specific help you need most right now.
Managing caregiving costs can be difficult and add significant stress. Our program can connect you with local organizations that offer financial counseling such as preventing utility shut-off due to debt, finding emergency aid, or learning how to balance your budget and debt
